Remember that the MSHSAA Pre-Participation Physical must be on file in the Athletic Office before you can begin tryouts for any sport! These forms are available on this website and in the school office. Please mail or stop by to turn in your forms before Febuary 25th. This will help expedite the paperwork and allow your daughter to participate the first day. Please mail the forms to Notre Dame High School- Attn: Mr Bayens or drop them off to Mr. Bayens' mailbox.
